Powered by a team of healthcare professionals, Polaris Care Delivery is a holistic care program that is geared towards improving your health and wellness through self-managed care advice and community resources. Your personal Care Navigator will work with you to provide medical advocacy, support, and other resources to keep you going.

Whats included in your membership:

• A Comprehensive Interdisciplinary Assessment and written report
• 1 hour of care navigation per month
